Confirmation of Attendance

At your request, and with your written permission, we will send an email report of your counselling attendance.  This report will contain all attended appointments, but will not contain any information related to topics discussed or your personal history.  For security reasons, this information can be sent only by email to a specific person.  Please allow 48 hours notice (miniumum) when requesting such a report.

NOTE REGARDING DOCUMENTATION FOR DEFERRED EXAMS:  Students who are not current clients of Counselling Services and are seeking documentation for a deferred exam are asked to contact their faculty or department regarding deferred exam procedures and documentation.
