Documentation Requirements |

Students requesting academic support services from the Accessible Learning Centre for classroom or examination accommodation are required to self-identify and provide current documentation from a registered health-care professional (i.e., physician, psychologist or psychiatrist).
Disability Documentation Deaf/deafened/hard of hearing Audiologist report Blind or visually impaired CNIB registration number or ophthalmologist report Medical or physical condition Letter from family physician or specialist Acquired Brain Injury Medical letter or neuropsychological assessment Mental Health Letter from family physician, psychologist, psychiatrist, or a psychiatric consultation. Learning Disability or ADHD Psycho-educational assessment completed within the last 3-5 years. Interim documentation- IEP/IPRC is acceptable for first-year students.

On receipt of this documentation, and after meeting with the student to determine their needs, the student will be formally registered as a client of the Centre. Students with learning disabilities and/or ADHD must present a current psycho-educational assessment performed under the supervision of a registered psychologist to access services.
Examination accommodations are determined by the Accessible Learning Centre’s Disabilities Consultants, based on recommendations set out in the student’s documentation. Recommendations involving facilities issues may be referred to the Manager of Accessible Learning, who, in conjunction with relevant university departments, such as Physical Resources, will determine an appropriate course of action.
